2019 Community Health Symposium and Sapphire Awards

REGISTRATION FEES

Following are registration fees for the Symposium. Registration options include the Full Symposium (two days of activities), a single day Symposium seat, or Sapphire Awards Program and Dinner only. Registration closes on May 3, 2019, one week prior to the Symposium.

Early Registration: October 1, 2018 – November 30, 2018

Full Symposium and Sapphire Awards DinnerMay 9 and May 10$90.00*

*Early registration pricing applies to the Full Symposium and Sapphire Awards Dinner. Register before November 30, 2018, to receive two days of Symposium activities for $90.00!

Regular Registration: December 1, 2018 – May 3, 2019

Full Symposium and Sapphire Awards Dinner May 9 and May 10 $135.00 
Thursday - Symposium and Sapphire Awards Dinner May 9 only  $95.00 
Sapphire Awards Dinner only  $80.00 
Friday - Symposium  May 10 only  $75.00 

CANCELLATIONS, REFUNDS, AND SUBSTITUTIONS

In the event you are no longer able to attend the Community Health Symposium and Sapphire Awards, we encourage you to transfer or request a substitution for your registration.

Should a cancellation be requested, a partial refund may be granted for cancellation requests made prior to March 15, 2019. A cancellation fee of $25 will be assessed to cover administrative costs. Substitutions are accepted without penalty. Registration fees will not be refunded for any cancellation request received after March 15, 2019.
