Welcome to St. Joseph's University Medical Center's Allied Health Student Registration website.  
St. Joseph's is one of the largest teaching hospitals in Northern New Jersey. According to this strong tradition, we are proud to offer educational opportunities for students to complement their training as future allied health professionals. 
 
This site is designed for faculty/coordinators to register themselves and their students for their rotations. The Faculty/Student Documents and Attestation Signature Page 3 must filled-in and uploaded for each faculty and student to complete the registration.  

The StudentFormsTotalandSignaturePage (SP) link in the Downloadable Files Section below contains the: (1) Confidentiality Statement,
(The Critical Facts Reference Guide - download from the second link below -  is to be read prior to completing the Confidentiality Statement), (2) Assumption of Risk Form, (3) Code of Conduct form, (4) Faculty/Student Documents and Attestation Signature Page and the N-95 Mask (Model 3M Aura 1870 & N95) Fit Testing Confirmation (Note: Fit testing is the responsiblity of the educational institution). (Click the SP link below in the Downloadable File Section to download them.)  The forms should be read and the Faculty/Student Documents and Attestation Signature Page 3,  signed and uploaded separately.

Note to Faculty
  •  
    Your registration confirms that all students and faculty meet the following requirements:

    Validation of Requirements

    Immunizations: Negative 
    • A negative two-step PPD skin test for Tuberculosis (or one PPD test if proof of one negative PPD test within the last year is submitted) or a negative TB Blood Test such as Quantiferon-TB Gold or T-Spot TB Test. 

    If PPD Test or TB Blood Test is positive, a negative Chest X-ray is required                                                             
    Diagnostic laboratory testing which includes Titers for:

    •  Rubeola, Rubella, Mumps                                                                                          
    •  Varicella                                                                                                                      
    •  Hepatitis B Core Antibody,                                                                             
    •  Hepatitis B Surface Antigen and                                                                                 
    •  Negative test results for National Institute of Drug Abuse standard panel urine drug screen 
    Influenza Vaccination ~ mandatory during Flu season

    Student fit tested for St. Joseph's Health, Inc. approved N-95 mask. (Completed by school)

    Background Record
    • Criminal Background Check completed within the last year and has been reviewed for statewide criminal history and Social Security number validity.  If positive, original report must be sent to Director of Clinical Education.

    Students and faculty scheduled in dialysis-related patient care areas:

    •  Hepatitis C Antibody and Hepatitis B Surface Antibody (unless documentation of immunity to Hepatitis B is provided)
    •  Hepatitis B Vaccination/Declination Form and a negative blood test for active disease.

    Maternity Record

    •  Pregnancy Test ONLY for female students assigned to Radiology areas
